Team Leader & In-Home Caregiver (Residential Habilitation & Supported Living Services)

A Team Leader/House Coordinator is an experienced, professional caregiver who has demonstrated an ability to support and care for co-workers, as well as for individuals with intellectual and developmental disabilities. A Team Lead understands and embodies the Company's mission, and actively incorporates our core values (Independence, Individuality, Inclusion, and Integrity) in their daily work.
Team Leaders serve as the first line of Direct Support Professionals for our wonderful Clients, fostering service and development, and ensure that employees who are assigned under their stewardship feel supported, heard, validated, and well-trained. Team Leaders will be assigned to oversee the quality of care of a specific home, with specific Clients and employees.
Qualifications
- Be at least eighteen (18) years of age
- Be a high school graduate, have a GED, or demonstrate the ability to provide services according to a plan of service
- Must pass a criminal background check and drug screening
- Be certified in first-aid and Cardio-Pulmonary Resuscitation (CPR) **We can help you obtain this certification!**
- Be certified to pass medications (Assistance with Medications) **We can help you obtain this certification!**
- Possess a valid driver’s license and vehicle insurance
- Previous caregiving experience; working with individuals with intellectual and/or developmental disabilities is a plus!
- Excellent verbal and written communication skills
- Ability to work collaboratively with a diverse team
- Flexibility to work a variety of shifts, including weekends and holidays
- Working knowledge of computers, and ability to complete digital charting in a timely and accurate manner
Responsibilities
- The Team Leader is responsible for all activity in the home
- Be a leader to the team members in the home under your charge. All team members in the home will report to you
- Complete daily charting checks to ensure all team members within the home have completed the required charting (MARs, ADLs, Goals, Client Time, Etc.)
- On-call 24/7 for the specific home you work in; answering behavioral-related calls from team members, de-escalate or do crisis intervention with Participants, filling shifts for the home you work in, when required
- Working with Regional Scheduler to problem solve scheduling issues within the specific home you work in – filling the vacant shifts as needed
- Notify the nurse of any medication discrepancies
- Coordinate with QIDP and regional nurse on all behavioral and medical needs for Participant(s) within the home including: appointments, meetings, finances, activities, etc.
- Ensure client/house binders are stocked with necessary forms
- Act as a liaison for employees and upper management regarding concerns, suggestions for improvement, etc.
- Create and implement chore charts, weekly menus, and activities schedule.
- Assist Clients with shopping needs for groceries and household items
