Community Based Therapist at Hoyleton Youth and Family Services – Fairview Heights, Illinois

Salary Range: $52,624.00 - $65,780.00 per year

Hoyleton is seeking a community-based clinician who is comfortable stepping outside traditional office walls and into the real environments where change happens- schools, homes, and community spaces. This role serves primarily children and adolescents within diverse, underserved populations, with assignments that may include adult clients based on program needs and clinician qualifications.

This is a hands-on, field-driven position for someone who can balance clinical care, documentation, scheduling, and collaboration without constant oversight.

What You’ll Do

You’ll serve as both clinician and connector, delivering high-quality therapeutic services while coordinating care across families, schools, agencies, and internal teams.

Direct Clinical Care

• Provide individual, group, and family therapy using evidence-based and developmentally appropriate approaches
• Deliver services in office and community-based settings including homes, schools, and partner locations
• Conduct diagnostic assessments and develop Integrated Assessment and Treatment Plans (IATPs) with clients, families, and caseworkers

Crisis Support & Case Coordination

• Provide crisis intervention services when needed
• Schedule and participate in clinical case staffing to ensure consistent communication across all care partners
• Collaborate with external agencies and multidisciplinary professionals to coordinate services

Documentation, Compliance & Quality

• Maintain timely, professional case records and clinical notes
• Ensure accurate billing and documentation in compliance with Medicaid Rule 132 and Managed Care Organization requirements
• Participate in utilization reviews and internal quality processes
• Complete all required reports and client documentation within established timelines

Professional Growth & Team Engagement

• Participate in weekly clinical supervision focused on strengths, development, and clinical effectiveness
• Contribute to a fast-paced, collaborative, multidisciplinary team environment

Who We’re Looking For

This role is for a clinician who combines clinical skill, cultural awareness, and personal accountability, and who can move confidently between therapy, documentation, and collaboration.

Required Qualifications

• Master’s degree in Social Work, Counseling, Psychology, or related human services field
• Strong organizational, time management, and communication skills
• Demonstrated ability to work effectively with diverse populations and cross-disciplinary professionals
• Knowledge of evidence-based practices and treatment modalities for the populations served
• Valid driver’s license and proof of auto insurance
• DCFS/CANTS clearance eligibility

Preferred

• LCSW or LCPC licensure
