About This Position

Nexus-PATH is located across North Dakota and offers mental health services, outpatient clinical services, and treatment foster care.


We have an opportunity to hire a full-time Community-Based Case Manager in our Bismarck, ND Nexus-PATH office!


This role will be supporting our brand new Therapeutic Host Homes Program!

Therapeutic Host Homes are family-style living arrangements for children ages birth – 18 with intellectual or developmental disabilities (ID/DD). A child lives with a caring provider in their community surrounded by a circle of support, provided by Nexus Family Healing, instead of living in a group home or institution. This is a voluntary program designed to help children return to their biological families when possible while continuing to receive help and mentorship from their Therapeutic Host Home provider.

Schedule/Pay/Location:

  • On-site (community based) in Bismarck, ND
  • Some flexibility in scheduling based upon client needs
  • Monday-Friday work schedule
  • $48,000-$53,000 starting salary

RESPONSIBILITIES:

  • Recruit, license, train, supervise, and provide ongoing assessment to resource families to ensure quality care consistent with regulatory agencies (i.e. DHS, COA, FFTA).
  • Participate in the development and implementation of local, regional and state plans for recruiting and retaining resource families.
  • Follow state and agency guidelines for conducting family/homestudy, physical environment inspection and criminal background checks and provide required documentation for initial and ongoing licensing.
  • Coordinate required orientation and ongoing education and skill development for resource families.
  • Supervise resource families’ interaction with foster child, child’s family, treatment team members and collateral contacts.
  • Monitor and evaluate resource families’ activities to ensure compliance with agency and state standards. Submit critical incident reports and develop and implement corrective action plans as required.
  • Facilitate required team/unit meetings and participate in other resource families related activities.
  • Provide coordination of all tasks required to meet the treatment needs and best interests of service recipients
  • Complete a systemic assessment of the needs of the child and family.
  • Establish service parameters with legal custodian as child’s needs dictate.
  • Provide complete and required documentation, within established time lines relating to case management and treatment planning.
  • Implement treatment services to meet the needs and goals of the children and families utilizing quality case management practice to provide for the best interest of the child and family.
  • Provide direction to other team members when applicable and make recommendations for discharge, finalization and permanency.
  • Advocate for the development and maintenance of funding and resources to meet youth and family needs at local, state and national levels.
  • Develop and maintain community contacts to promote Nexus-PATH and collaborative efforts.
  • Provide crisis intervention as needed to maintain safety and provide for the best interest of the child and foster family.
  • Integrates trauma informed care principles into practice by sharing knowledge and understanding of trauma with the child, foster family and relevant team members.
  • Routinely screens for trauma exposure and related symptoms and demonstrates competence in using screening tool results to enhance service provision.
  • Routinely makes resources available and shares knowledge with foster families and providers on trauma exposure, its impact, and trauma-focused treatment.
  • Engages in efforts to strengthen the resilience and protective factors of children and families impacted by and vulnerable to trauma.
  • Emphasizes continuity of care and collaboration across child-service systems.
  • Recognize and value cultural differences in all aspects of work and service delivery
  • Understand the potential for secondary traumatic stress and practices self-care strategies to mitigate these effects.
  • Meet standards for productivity and caseload size while maintaining appropriate documentation and files

QUALIFICATIONS:

  • Bachelor’s degree in Social Work preferred, Bachelors in Human Service Degree required
  • Current ND social work licensure required (LBSW) or ability to gain one within 3 months
  • Experience in foster care, adoptions and/or family-based service preferred
  • Valid driver’s license required. Must meet state regulating agency driving requirements, maintain auto insurance and pass corporate insurance eligibility standards
  • Must have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges.
  • Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values.
