Description

Compensation $68,250.00 per year

Schedule: M-F 830a-430p plus traige on call responsibilities.

Medical or medical stipend. Dental, Vision, Benefit time, 401K.

Leadership & Supervision

  • Supervise and mentor Intake Coordinators and support staff, providing training, guidance, and performance evaluations.
  • Develop and maintain staffing schedules to ensure adequate coverage for intake operations.
  • Foster a positive, team-oriented environment that promotes accountability and professional growth.

Operational Oversight

  • Oversee all hospice and palliative care referrals, ensuring timely evaluation and admission decisions in compliance with organizational policies and Medicare Conditions of Participation.
  • Monitor and ensure accuracy of patient registration/intake, orders, and documentation in the EMR system.
  • Review daily census reports and coordinate with Billing and Clinical teams to ensure accurate data flow.

Quality & Compliance

  • Ensure adherence to HIPAA regulations and organizational confidentiality standards.
  • Participate in Quality Improvement initiatives, analyze intake data, and recommend process improvements.
  • Maintain compliance with state and federal regulations governing hospice care.

Communication & Coordination

  • Serve as the primary liaison between referral sources, physicians, and internal teams for escalated intake issues.
  • Provide feedback to referral sources and ensure exceptional customer service standards.
  • Respond to complex inquiries regarding eligibility and available services.

Other Duties

  • Participate in committees and organizational meetings as assigned.
  • Serve as backup for the Intake Coordinator role, and perform additional duties as assigned based on business needs.
  • Cover on-call triage responsibilities as needed.
  • Participates in all required in-service education programs and meetings.
  • Adheres to all employment policies.
  • Demonstrates compliance with the dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.
  • Performs other duties as assigned.

Position Requirements

Required Skills/Abilities:

  • Knowledge of the dying process; knowledge of policies and procedures of the hospice organization.
  • Knowledge of regulations surrounding compliance with Medicare Conditions of Participation.
  • Ability to function as a resource to patients and families during times of high stress and volatility.
  • Ability to assess patients to determine appropriateness of participation in hospice.
  • Ability to build constructive relationships with colleagues, and outside agencies, organizations, and individuals.
  • Excellent communication skills to effectively deliver information between referral source and the hospice clinical team.
  • Consistently model respect for, and accountability to, the public, patients/families, and co-workers. Communication and actions reflect care, courtesy, honesty, and professionalism.
  • Convey a professional and positive image and attitude regarding the hospice organization.

Education and Experience:

Required Education and licensure: Associate degree in a related field. Licensed practical nurse; New Jersey and Pennsylvania LPN License.

Preferred Education:

Required Experience: Two (2) years of experience in a Medicare-certified Hospice/Home Care agency with at least 1 year in a leadership or supervisory role. Evidence of leadership ability; including evidence of ability to take initiative, delegate authority appropriately, ability to role model, ability to serve as a change agent.
