Physical Therapist (PT) at Parkview Health – Fort Wayne, Indiana

Provides high-quality patient care and physical therapy services by evaluating, planning, implementing, and documenting treatment. Assesses and treats patients in accordance with physician orders, utilizing various therapeutic procedures and assistive devices to optimize functional potential in all areas of the patient's life. Develops and implements individualized treatment plans based on patient needs, establishing achievable functional goals. Regularly evaluates and updates treatment plans as needed throughout the course of care.

Collaborates with and provides appropriate supervision for Physical Therapist Assistants (PTAs), ensuring effective execution of treatment procedures. Educates and instructs patients, families, and caregivers on exercises and therapeutic home programs to support continued progress post-discharge. Completes all required documentation and billing promptly and accurately. Care is delivered in compliance with the hospital’s philosophy and essential functions, as well as the standards set by the rehabilitation department, Indiana State Practice Act, American Physical Therapy Association, and The Joint Commission.

• Must be a graduate of an accredited physical therapy program with the following degree requirements:
  • Minimum of a Bachelor’s degree if graduated before January 1, 2016.
  • Doctorate of Physical Therapy (DPT) required if graduated after January 1, 2016.
  • International candidates must have graduated from an accredited program with at least a Bachelor’s degree in Physical Therapy (DPT preferred).

• Must hold or be eligible for a Physical Therapist license in the state of practice (to be obtained within 90 days of employment).
• CPR certification required within 90 days of employment.
• Must maintain all required licenses and certifications.
• American Physical Therapy Association membership preferred.
• If assigned to drive a company vehicle, a valid driver’s license is required.
• If using a personal vehicle for work, proof of insurance is required along with a valid driver’s license.

• Work experience is not required beyond clinical experience obtained during an accredited training program.

• Must be able to lift 50 pounds, assist with 100 pounds, and push up to 300 pounds.
• Strong verbal and written communication skills required.
• Must be flexible and willing to work at other locations as assigned.
