About This Position

Description:

SUMMARY: The Care Coordinator will follow the Wraparound process to provide care coordination to identified children, youth & their families. The C.A.R.E.S. Wraparound model is a strengths-based, family driven diversion program designed to divert youth and families from the child welfare system, juvenile justice system or residential care.

Essential Duties and Responsibilities:

  • Facilitate Family Team Conferences and follow the Wraparound process to manage caseload
  • Build relationships with family members, providers and the community
  • Promote family voice and choice in work with families and community providers
  • Practice trauma informed care
  • Complete all necessary documentation to ensure Wraparound model is followed with fidelity
  • Perform other incidental or related duties as assigned

Employee Responsibilities:

  • Follows policy and procedures on annual TB testing.
  • Follows policy and procedures on annual influenza vaccinations.
  • Attends all mandatory in-services.
  • Attends and participates in scheduled training and educational classes to maintain current certifications in accord with the State of Montana.
Requirements:

EDUCATION/EXPERIENCE:

  • Bachelor’s degree in social work or related field
  • Knowledge of mental health systems of care and support services in Yellowstone County
  • 2 years of experience working with children, youth and families that have complex needs and are multi-system involved

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Excellent written and verbal communication & interpersonal skills
  • Demonstrate cultural competence & sensitivity to population served
  • Proven ability to work with diverse groups and build strong partnerships
  • Support and uphold the mission, beliefs and values of St. John’s United
  • Proficient in the use of computers and Microsoft office programs and virtual web platforms

TRAVEL/OTHER:

  • Candidate must have valid driver’s license, proof of current insurance and be able to drive throughout the state of MT. Other national travel may be required.
  • Must be able to pass a thorough background screening upon hire.
  • Some occasional evening and weekend hours required

PHYSICAL DEMANDS:

  • Requires minimal lifting functions
  • Requires moderate bending, standing and reaching activities

WORKING CONDITIONS:

  • Normal office setting. No hazardous material exposure
  • This position requires the ability to function independently in performing assigned duties
