Position Summary:
We are looking for a compassionate and energetic Home With Meds Representative to join our team. This individual must have a warm, trustworthy demeanor and an engaging voice that fosters patient confidence and comfort. The ideal candidate will build strong relationships with patients, ensuring consistent communication and adherence to their medication schedules while maintaining accurate records. Must be a licensed pharmacy technician in the State of Indiana.

Key Responsibilities:

• Conduct first communication within 24-48 hours of discharge from skilled nursing.
• Conduct monthly patient outreach calls to confirm medication delivery dates.
• Engage patients in a natural, conversational manner while reviewing the required questionnaire, ensuring it does not feel scripted.
• Identify and escalate trigger events to the assigned sales representative for appropriate follow-up.
• Act as a bridge between the patient and the pharmacist, facilitating communication when needed.
• Build and maintain patient relationships to improve adherence to monthly calls and encourage open, honest communication.
• Accurately document all patient interactions.

Qualifications & Skills:

• Kind, empathetic, and able to quickly build trust with patients.
• Excellent verbal communication skills with a clear, engaging, and friendly tone.
• Strong ability to listen actively and respond appropriately to patient needs.
• Comfortable handling sensitive medical information with professionalism and confidentiality.
• Detail-oriented with the ability to accurately document information.
• Proactive in identifying concerns and escalating them to the appropriate team members.
• Prior experience in a call center, customer service, or healthcare-related role is a plus.

Qualifications:

• Licensed Pharmacy Technician in the State of Indiana

Preferred Qualifications:

• Familiarity with CRM systems and patient communication best practices.

Experience in a pharmacy or healthcare setting is beneficial but not required
