About This Position
The mission of Correctional Managed Care is to address the healthcare needs of underserved patient populations within the Texas Department of Criminal Justice (TDCJ) and the Texas Juvenile Justice Department (TJJD).

JOB SUMMARY:
The Clinical Case Manager is responsible and accountable for coordinating the care of a specified group of patients.
ESSENTIAL JOB FUNCTIONS:
- Functions as a resource to nursing staff regarding patient assessment and plan of care or performs patient assessment and develops a plan of care to assure consistent and appropriate care is provided in a patient focused manner.
- Collaborates with the physician and staff to develop, implement, and revise critical paths while ensuring the initiation of individualized critical paths for each patient entering the system and monitors patient progress.
- Analyzes and addresses aggregate variances as well as variances from individual patients and shares this information with staff, physicians, and administration.
- Educates the staff on case management and provides specific information related to case types, including financial data.
- Develops and implements a documentation system incorporating critical paths and nursing care plans into a concise, multi-disciplinary format that meets regulatory standards.
- Plans and implements (through multi-disciplinary meetings or rounds) strategies to reduce length of stay, reduce resource consumption, and achieve positive patient outcomes.
- Utilizes appropriate resources in cases that present ethical dilemmas.
- Manages each patient's transition through the system and transfers accountability to the appropriate person upon entry into another clinical service or upon discharge.
- Adheres to internal controls and reporting structure established for department.
- Performs other related duties as required.
EQUIPMENT:
- Standard patient care supplies, equipment, and mechanical devices. Standard office equipment.
WORK ENVIRONMENT:
- Located within the confines of a prison, jail or juvenile detention center.
- Conditions such as noise, odors, cramped workspace and/or fumes could sometimes cause discomfort.
- Mild to moderate physical effort may be required such as walking, standing and lifting materials, equipment, objects, and/or patients.
- Occasional lifting may be heavy and awkward, over 50 lbs.
- Vision, hearing, talking, and sense of touch abilities must be adequate enough to enable one to quickly and accurately perform tasks such as: reading small print, reading from monitoring equipment, defining details, sending and receiving clear and accurate verbal communication.
- Frequent periods of concentrated or focused attention will be needed to interpret visual, auditory, and sensory inputs.
- Shift work may be required.
- Alertness and careful attention to detail will be required to avoid injury.
- May be exposed to such occupational hazards as communicable diseases, radiation, chemotherapeutic agents, and disoriented or combative patients.
- May be required to be on standby and work extended shifts in situations deemed necessary by a designated Executive Director and/or Nursing Director.
Security clearance is required; pre-employment drug testing is also required.
REQUIRED EDUCATION/EXPERIENCE:
- RN with Bachelor's degree in nursing or related field and four years clinical experience, with at least one year of this with a specific patient population.
- Current license or valid permit to practice professional nursing in Texas.
Salary is commensurate with years of relevant work experience.
