Under the direction of the Medical Director or Nurse Manager, the Clinical Nurse Specialist - Group Homes – Group Homes oversees and supervises the medical care and medication administration in Level-3 licensed residential programs to meet State regulations for training, observation and oversight.

MAJOR RESPONSIBILITIES:

• Provide medication administration oversight.
• Review and update Group Home Policies and Procedures regarding medication administration.
• In person nursing assessment of new admission.
• Review medical appointments and update EMR as needed.
• Review and update Nursing Care Plans.
• Provide staff training for any special procedures and general care of clients.
• Assess each individual client weekly.
• Identify concerns and work with teams to find appropriate care/solution.

Qualifications: Qualifications

Job Requirements

• Graduate of an accredited nursing program with an active nursing license in the State of Vermont
• Minimum of 5 years of nursing experience
• Proficiency in Microsoft Office (including Outlook)
• Experience with electronic health record (EHR) systems
• Strong written communication skills
• Valid driver’s license
