JOB SUMMARY
The Occupational Therapist provides evaluation and treatment services for short-term rehabilitation patients and long-term care residents experiencing illness, injury, or age-related functional decline. Working under the supervision of the Rehabilitation Manager and Director of Rehabilitation, the OT develops and implements individualized, patient-centered therapy plans focused on improving, maintaining, or slowing the progression of functional limitations, consistent with Campbell County Health’s mission and values.
Responsibilities include supporting admissions and discharges for short-term rehab patients, completing required screenings and follow-up assessments, addressing changes in condition to reduce loss of function, and communicating progress toward therapy goals with residents, families, and the interdisciplinary team. The OT maintains accurate documentation, participates in program development and performance improvement initiatives, and pursues professional growth to meet the needs of the skilled rehab and long-term care population.
Essential Functions
  • Assess short-term rehab patients and LTC residents, reviewing medical history, administering standardized and informal tests, identifying deficits, and establishing individualized, patient-centered plans of care in the EMR.
  • Develop, update, and modify plans of care addressing physical and psychosocial needs in collaboration with the healthcare team; document progress, barriers, and interventions.
  • Collaborate with OT Assistants, Rehab Aides, and students, maintaining supervisory documentation per WYOT regulations.
  • Complete timely, accurate documentation of skilled treatments, CPT coding, and EMR entries within 24 hours of service.
  • Conduct comprehensive screens for LTC residents as needed, quarterly, or annually to prevent functional decline; notify POAs and communicate therapy plans and progress.
  • Educate residents and families on evaluations, therapy plans, goals, and outcomes; provide training and opportunities for observation as appropriate.
  • Participate in interdisciplinary care planning, OPCs, and rehab team meetings to coordinate resident care.
  • Provide supervision, guidance, and support to staff and students to ensure safe, efficient, and compliant care.
  • Maintain professional standards, including licensure, continuing education, fiscal responsibility, and adherence to CMS, NBCOT, WYOT, and CCH policies.
  • Demonstrate service excellence, positive interpersonal relations, and efficient time management.
  • Maintain a clean, safe environment conducive to quality care.
  • Work scheduled hours, including weekends/holidays as needed, and provide coverage across CCH rehab services.
  • Adhere to Campbell County Health’s Corporate Compliance Program, including the Code of Conduct and applicable laws and regulations.
  • Perform additional duties as assigned.
JOB QUALIFICATIONS
  • Education
    • Graduate of an approved/accredited occupational therapy school.
  • License
    • Must currently be licensed in, or be eligible to be licensed, to practice occupational therapy in the state of Wyoming. Current license holders must be in good standing with the Wyoming Board of Occupational Therapy.
  • Certifications required
    • See Cardiopulmonary Resuscitation Certification Policy and Certifications/Education Requirements Policy.
  • Experience
    • Graduate from an accredited AOTA occupational therapy program.
