About the Role:  As an advocate, you will be a trusted guide and support for the clergy members of the Catholic Archdiocese of Denver, helping them to navigate the complexities of healthcare and supporting their overall well-being. You will coach members in making positive and lasting lifestyle changes, improve their nutritional choices and encourage them to meet individual goals.  Rather than providing hands-on clinical care, you’ll serve as a compassionate coordinator—ensuring that clergy members receive the right care, in the right place, at the right time.

Key Responsibilities:

Serve as a dedicated advocate and primary point of contact for clergy, supporting them in navigating healthcare services, benefits, and resources.

  • Provide compassionate, faith-respectful support aligned with the mission of serving clergy

  • Coordinate medical, dental, and specialty appointments to ensure timely access and continuity of care

  • Support clergy with insurance navigation, prior authorizations, claims questions, and benefit coordination

  • Provide clear, accessible health education so members understand and fully utilize their benefits and resources

  • Promote preventive wellness by educating members on health initiatives and available programs

  • Conduct regular check-ins and proactive outreach to identify needs early and prevent care gaps

  • Create individualized care plans tailored to each member’s physical, emotional, and practical needs

  • Collaborate with pharmacists, social workers, and care managers to deliver coordinated, whole-person care

  • Provide post-hospitalization follow-up and optional on-site visits when appropriate

  • Assist with medication coordination and communication between providers

  • Assess nutritional needs and encourage healthy lifestyle changes

  • Assist members with remote monitoring tools such as glucometers, blood pressure cuffs, and medical alert systems

  • Manage delivery of medical supplies and other essential health-related items

  • Conduct home safety assessments and arrange modifications, meal services, or support services as needed

  • Connect members with community resources including transportation, home care, and social support services

  • Develop and support outreach efforts to increase awareness and engagement in available services

  • Participate in client and team meetings to identify needs and coordinate comprehensive support

  • Maintain accurate and timely documentation of member interactions and care coordination activities

  • Maintain strict confidentiality and compliance with PHI and HIPAA guidelines

  • Engage respectfully and professionally with clergy, families, and colleagues

  • Travel locally for member visits, diocesan events, or meetings as needed

  • Perform additional duties as assigned by the Director of Care Management

Who We’re Looking For:

  • Minimum 3 years of experience in healthcare coordination or a related support role.
  • RN or LPN license (active or inactive). Ideal for nurses seeking a meaningful non-bedside role.
  • Exceptional communication and listening skills with the ability to offer emotional support.
  • Strong organizational abilities and attention to detail.
  • A humble, service-oriented demeanor and genuine desire to support others.
  • Ability to work independently and collaboratively with healthcare professionals.
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint).
  • Experience in clinical or medical office settings and a passion for helping individuals navigate the healthcare system
  • Able to travel within the geographical territory of this Archdiocese. Travel expenses will be covered.


Compensation & Benefits:

  • Salary will be commensurate with experience and qualifications with a range between $25-$27 per hour.
