About This Position
JOB SUMMARY
The High-Risk Care Management Coordinator is responsible for supporting care management activities for high-risk patients with complex medical, behavioral health, and social needs. This role assists in coordinating care services, facilitating communication between patients and providers, and ensuring timely access to appropriate healthcare and community resources.
The High-Risk Care Management Coordinator supports Care Managers, nurses, and interdisciplinary teams in managing patients identified as high-risk through risk stratification, utilization patterns, or chronic disease burden. The role focuses on improving care coordination, promoting patient engagement, reducing avoidable hospitalizations and emergency department utilization, and supporting continuity of care.
This position plays a key role in identifying barriers to care, facilitating transitions of care, and assisting with interventions designed to improve health outcomes and quality of care for high-risk populations within a California Managed Services Organization (MSO).
Requirements:MINIMUM & PREFERRED QUALIFICATIONS
Education/Training
Minimum: High school diploma or equivalent required
Preferred: Associate’s degree or higher in healthcare administration, nursing, public health, social services, or related field. Certification as a Medical Assistant or Care Coordination certification.
Experience
Minimum: At least two years of experience in healthcare coordination, care management support, medical office operations, or related healthcare setting.
Preferred: Experience supporting high-risk or complex patient populations. Experience in care management, population health, or managed care programs. Experience working in MSO, IPA, or health plan environment. Experience with Medicare Advantage, Medi-Cal, or managed care populations. Experience using electronic health records or care management systems

Work is performed primarily in an office or remote setting. Requires prolonged sitting, frequent computer and telephone use, and the ability to review documentation and coordinate care activities. Occasional standing, walking, or local travel may be required. Ability to lift and carry work-related materials up to 15 pounds. Requires strong attention to detail, organizational skills, and the ability to function in a fast-paced healthcare environment.
