Summary
The Home Health Certified Nursing Assistant provides direct and indirect care to patients in their residence as directed by the plan of care and under the supervision of a Registered Nurse in accordance with the philosophy, goals, and objectives of Cozad Community Health System.
Essential Duties and Responsibilities

· Participates in, monitors, and implements the patient’s plan of care.
· Recognizes changes in the patient’s condition and notifies the primary RN in a timely manner.
· Demonstrates expertise in provision of daily activities for patients and delivers such care in a professional manner.
· Provides care that includes, vital signs, positioning, transferring, toileting, nutrition, ambulation, bathing, grooming, dressing/undressing, hygiene procedures, and linen changes.
· Performs and maintains documentation.
· Follows all HIPAA guidelines and promotes and ensures patient safety.
· Special projects and other duties as assigned.

Requirements:Qualifications

Education and/or Experience
High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience. Electronic Health Records experience preferred.
Language Skills
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively to patients, visitors and employees.
Mathematical Skills
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability
Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.

While performing the duties of this Job, the employee is regularly required to sit; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is occasionally required to stand and walk. The employee must regularly lift and/or move 10 pounds must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
