Title: RN Multiple Locations Site Manager Job Classification:
Job Summary: The RN multiple locations site manager is a registered professional nurse who ensures quality of patient care in the home. Primary function is for the overall supervision of patient care and monitoring of appropriate staffing and productivity. Coordinates care with the interdisciplinary group, patient, family and referring Hospice. Facilitates communication between the multiple locations site and the Parent office. Reports to: (per Hospice) Regional Clinical Director

Job Qualifications:
Education: Graduate from an accredited diploma, associate, or baccalaureate nursing program. Licensure: Active, unencumbered state license as a registered nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state. Current driver’s license.
Experience: One year of experience as a registered nurse in a clinical care setting. Home health experience preferred.
Skills: Nursing skills as defined as generally accepted standards of practice. Excellent interpersonal skills and ability to communicate effectively. Demonstrates proven decision-making skills. Must read, write, and comprehend English.
Transportation: Reliable transportation and valid auto liability insurance. Travel to multiple locations required.
Environmental and Working Conditions: Works in patients’/clients’ homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls.
Physical and Mental Effort: Prolonged standing and walking required, with ability to lift up to fifty pounds and move patients/clients. Requires working under some stressful conditions to meet deadlines and patient/client needs, and to make quick decisions and resource acquisition; meet patient/client and family individualized psychosocial needs. Requires hand-eye coordination and manual dexterity.
Essential Functions: Directs and coordinates patient care Assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations. (Conditions of Participation)
Participate in activities relevant to professional services including development of Hospice personnel.
Supervises and provides direction to subordinates, in an effort to ensure quality, compliance with Plan of Care and assessment and reassessment of patient’s needs and continuity of services by appropriate health care personnel.
Monitors QAPI Program and assures appropriate corrective measures are performed. Promotes compliance with all fiscal intermediary and/or other third-party payors.
Monitors systems and identifies problem areas to DON, both verbally and through written reports. Meets mandatory continuing education requirements of the Hospice/licensing board.
Promotes and educates regarding the concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes
Provide patient care when needed and participate in after hours on call for all locations.
