SHARED LIVING COORDINATOR

Pay rate: $21 per hour

We are hiring a Shared Living Coordinator to join our Shared Living team based in Lewiston, ME. This is a full time position with a Monday to Friday day time schedule. Shared Living is a housing and support service for people with intellectual and developmental disabilities (IDD) or autism, and the Shared Living Coordinator supports a group of Shared Living Providers who have decided to open their homes to caring for a participant, from the matching and placement process through to ongoing mentorship.

JOB DESCRIPTION SUMMARY: This position is responsible for assisting with the development, management, and oversight of multiple Shared Living Programs. This includes providing ongoing support, mentoring and training to the Shared Living Providers.

ESSENTIAL FUNCTIONS:

Support Shared Living Providers in all areas related to their responsibilities

Adhere to a regular schedule of monthly home visits

Fully understand and abide by the agency’s code of ethics

Ability to adequately write Individual Program plan narratives that effectively reflect the individual

Ensure all OADS and Mainecare requirements are adhered to as related to Shared Living

Participate in PCP meetings, Quarterly Shared Living Provider meetings/trainings, communicate concerns and report outcomes back to the appropriate supervisor in a timely manner

Participate in planning meetings to ensure appropriate goal development and implementation as it relates to the Shared Living programs

Complete all necessary documentation to ensure timely submission of all documents to stakeholders according to the standards of Creative Works

Communicate and interact effectively and professionally with participants, providers, family members, guardians, case workers, and other stakeholders

RESPONSIBILITIES:

Provide administrative support by managing participant files

Aid Shared Living Providers with the appropriate tools and support to effectively manage the responsibilities of their independent programs.

Assists with the development of new Shared Living Programs.

Maintain accurate and timely documentation of home visits, and other related contact with the Shared Living Providers.

Ensure compliance with all Creative Works policies and procedures.

Ability to remain supportive and empathetic in all situations.

Ability to remain flexible and quickly adapt to changing circumstances.

Must be able to problem solve and offer solutions to challenging situations.

Other duties as assigned.

PHYSICAL WORK TOLERANCES:

While performing this job the employee is regularly required to sit for extended periods of time.

Ability to travel.

QUALIFICATIONS:

High School Diploma or GED.

Ability to speak, read and write in English.

Valid driver’s license required.

Satisfactory driving record per agency’s insurance company standards.

Must maintain a professional demeanor at all times.

Must have or be willing to obtain DSP or CDS, CRMA, and other trainings as required.

Experience and knowledge of the Shared Living option preferred.

Problem solving skills and strategic planning highly desirable.

Must be able to travel to conduct home visits and attend meetings.

Excellent time management and organizational skills

Must have professional level writing skills to maintain documentation standards, reports, and any other related responsibilities.

Must be willing to participate in training for Shared Living Providers on an intermittent basis as needed

Progressively responsible experience in managing services/programs is desired.

Direct supervisory experience a plus.
