About This Position
Position Title: SIL Case Manager
Reporting To: FRP Director
POSITION SUMMARY: The SIL Case Manager assists young adults to successfully transition from foster care into adulthood. Working with minimal supervision and responsible for managing the daily operations of the program and addressing the various needs/goals of each young adult that has aged out of foster care. Maintain a high standard of ethical behavior and professionalism and function as a member of the Cumberland team.
QUALIFICATIONS:
- Must be 22 years old or older
- Bachelor’s degree in a related field with 1-year documented experience (Required)
- Providing support services to youth and young adults who may have experienced child abuse and/or neglect, are in foster care, are homeless, at-risk, have mental or behavioral health challenges
- Providing independent living services to youth and young adults ages 18-22
- Excellent verbal communication skills
- Excellent writing skills including the ability to produce comprehensive narrative reports
- Team-player who can also work independently
- Excellent organizational skills, time management skills, and computer application skills
- Proficient in Microsoft programs, specifically Word and Excel
- Must possess the ability to be adaptable, motivate clients, demonstrate social interaction skills, exercise patience, good judgment, confidentiality
- Successfully complete required training meeting the requirements to provide ECM
- Must consent and complete all pre-employment screening, including background checks required by TDFPS including DPS, FBI fingerprinting, and DFPS Central Registry
RESPONSIBILITIES AND COMPETENCIES:
- Oversee the progress of the clients’ goals and be an accountable partner
- Create Service Plans with the clients that include program required content
- Conduct more frequent visits with residents requiring Enhanced Case Management (ECM)
- Service Plan review every 6 months with a focus on how the client will do to meet his or her own needs and quarterly evaluations
- Consistently document meetings and activities of the clients, ensure monthly documents are collected for the client’s educational and work activities
- Responsible for client case records and up-to-date file copies as required
- Encourage and motivate clients to participate in age-appropriate and independent living activities
- Complete client assessments as needed
- Monitor and enforce program rules
- Maintain an active role in creating, editing, and implementing program policies
- Potentially participate in court reviews.
- Perform other assignments and duties as requested
JOB DUTIES SPECIFIC TO ENHANCED CASE MANAGEMENT
- All duties previously mentioned.
- Assist young adults needing ECM with scheduling, obtaining, and maintaining needed medical and educational needs, finding employment, or other services through community-based providers, agencies, or other organizations.
- Assist ECM residents in scheduling and arranging transportation.
- Assist ECM residents in improving their daily life skills such as cooking, money management, cleaning, and shopping.
- Create initial goals for the ECM residents to be completed by the 30th day of residency.
- Determine whether ECM services are no longer necessary or should be discontinued based upon the client’s progress

WORKING CONDITIONS/PHYSICAL REQUIREMENTS:
- Lift and carry up to 10 pounds, occasionally up to 20 pounds, infrequently up to 50 pounds
- Performs tasks that may involve standing or walking 75% of the day, sitting 25% of the day
- Ability to bend, stoop, reach and grasp as necessary
- Able to respond to telephones and other auditory stimulations
