Position Summary
The Health-Related Social Needs (HRSN) Program Manager provides leadership and oversight for NWU’s HRSN services. This role supervises Community Health Worker (CHW) staff, supports quality service delivery, and strengthens coordination with community partners and Coordinated Care Organizations (CCOs). The Program Manager helps ensure clients can access healthcare resources and navigate health and social service systems through services that are culturally and linguistically appropriate, responsive, and trauma-informed, consistent with program requirements.

Essential Duties & Responsibilities

  • Supervise CHW staff and oversee HRSN service provision.
  • Provide guidance on program rules, policies, procedures, and requirements.
  • Train and support staff on HRSN program-related activities.
  • Manage program databases, spreadsheets, and other data collection tools.
  • Liaise with Coordinated Care Organizations (CCOs) to support NWU HRSN activities.
  • Work with organizational leadership to prepare and submit HRSN reports in accordance with program requirements.
  • Support CHW staff in their duties and responsibilities as needed.
  • Maintain a client caseload comparable to approximately 50% of a full CHW caseload.

Additional Responsibilities
  • Use technology and software tools effectively.
  • Maintain records as required.
  • Participate in collaborative efforts to improve access to healthcare and social services, service coordination, and client experience across our region.
  • Other duties as assigned.

Minimum Qualifications
  • Community Health Worker Certification or ability to earn certification within 90 days of hire.
  • At least two years of social service or related administrative experience.
  • Experience working with clients from a range of cultural, linguistic, and socioeconomic backgrounds.
  • Understanding of the healthcare system and how to help clients find needed resources.
  • Strong communication and organization skills; ability to collaborate across teams and partner organizations.
  • Demonstrated ability to communicate respectfully and effectively with clients with varied communication needs.
  • Attention to detail and ability to track and manage program information accurately.
  • Proficiency in MS Office (or similar tools) and ability to learn new systems.
  • Valid Oregon driver’s license and ability to be insured under NWU’s insurance policy.

Preferred Qualifications
  • Bachelor’s degree in social services or related field.
  • Bilingual in English/Spanish or other languages.

Work Environment & Physical Requirements
This role is primarily office-based with occasional outdoor work for events or activities. Regularly requires communication (talking/hearing), computer use, and light physical activity. Occasional standing, walking, reaching, stooping, or similar movements may be required. Must be able to lift and/or move up to 20 pounds.
