I. Major Responsibilities:
1. Participates in the assessment of the patient’s health status by collecting and recording data from documented information, observation, patient/significant other interview, and other health professionals.
2. Participates in the nursing diagnosis and patient needs based on assessment data. Includes physical, psychosocial, cultural, educational, and spiritual needs as applicable.
3. Participates in the identification of expected patient outcomes/goals.
4. Participates in the initiation of discharge planning on admission in collaboration with the patient/significant other and the multi-disciplinary treatment team. Participates in the review and revision of the discharge plan as needed.
5. Documents planned patient discharge instructions and services on the discharge instruction sheet. Completes patient referral form nursing information for patients receiving post discharge services.
6. Provides input into patient or caregiver learning needs. As directed by the Registered Nurse, documents an educational plan that demonstrates consideration of the patients age, cognitive level, culture, and preferred method of learning, i.e., printed matter, videos, and/or demonstration.
7. Documents patient response to interventions/nursing care.
8. Includes patient and/or significant other in planning for care.
9. Administers all therapeutic interventions in a safe and efficient manner.
10. Administers medication accurately, observing patient response, as evidenced by documentation in the medical record and the lack of negative outcomes.
11. Performs limited I.V. therapy procedures.
12. Assists physicians with treatments, procedures, and special tests as required.
13. May delegate selected patient care activities to other licensed practical nursing staff and/or nurse’s aide. Delegation is appropriate to the level of training and competence of staff and the patient need.
14. Responds promptly, professionally, and calmly to emergency situations.
15. Interacts and copes effectively with diverse populations in stressful situations resulting from trauma and illness.
16. Demonstrates effective communications methods and skills, utilizing appropriate reporting channels.
17. Participates in the orientation of new staff and acts as a preceptor when assigned. Completes orientation paperwork for new staff as outlined in hospital procedures.
18. Completes all unit task sheets or checklists as required.
19. Follows hospital and department policies and procedures.
20. Actively participates in departmental performance improvement and training activities.
II. Position Qualifications:
License/Certification/Education:
Required:
1. Current licensure as a Licensed Practical Nurse in the Commonwealth of Massachusetts.
2. BLS Certification.
3. CPR Certification.
Preferred:

Experience/Skills:
Required:
1. Ability to perform detailed assignments with minimal supervision and work under pressure.
2. Knowledge of the nursing process for the delivery and management of patient care from admission to discharge.
3. Ability to create documentation, which is accurate, legible, and concise.
4. Ability to complete and maintain all required licensed practical nurse competencies.
Preferred:
1. Prior experience in direct patient care.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
