RN MDS Coordinator at Cassia – Apple Valley, Minnesota

Apple Valley Village Health Care Center is currently seeking an experienced Registered Nurse (RN) to join our MDS team! As an MDS Coordinator at Apple Valley Health Care Center, you will manage the clinical care of residents by assigning different tasks to nurses within the department. Our ideal candidate must have previous MDS experience, thrives in a fast-paced environment, excels in multitasking, and has excellent customer service skills.

Position Type: Full-Time, benefits eligible position including working a nurse management weekend rotation

Shift Available:
Days Monday - Friday 8:00 AM - 4:30 PM including a nurse management weekend rotation

Wage Range: $42 - $48 / hour depending on experience

MDS Coordinator Responsibilities:

• Oversee and complete MDS and care planning.
• Assist Health Plan Nurse with Medicare scheduling, monthly billing, charting, and issuing notices of non-coverage.
• Work in collaboration with Admissions staff, Health Plan Nurse, and Other Department Managers.
• Perform additional tasks as needed.

MDS Coordinator Qualifications:

• RN license in the state of Minnesota required.
• Minimum of 2+ years experience preferably in LTC.
• MDS experience required.
• Proficient in computer software.
• Verbal and written communication skills.
• Detail oriented.
