Job Overview:
RWJBarnabas Health is looking to add a qualified full-time Social Worker, MSW in Somerville, NJ. The Social Worker assists patients and their families to achieve the goals agreed upon with the care management team as it relates to an illness, disease or psychosocial needs.
Qualifications:
Required:
- NJ Licensed Clinical Social Worker or NJ Licensed Social Worker or New Jersey State Licensure for BSW or New Jersey State Licensure for MSW
- Prior experience in social work in Behavioral Health Unit in an acute care hospital
Required:
- Two (2) years of experience working in a Behavioral Health setting
- Certified in DBT (Dialectical Behavior Therapy) or CBT (Cognitive Behavioral Therapy) preferred or willing to obtain training and certification upon hire
Scheduling Requirements:
- Day Shift, 40 hours/week
- Full Time Schedule
Essential Functions:
- As a member of the multidisciplinary care team, the social worker engages in specific assessments or interventions as part of the care plan, assists with the coordination of care transitions (admissions and discharge planning for example) and engages with the patient in various settings such as their home, the practice, the hospital or post-acute facilities
- The Social Worker consults and collaborates not only with the patient's care team but also with other external agencies and community resources to secure support services, and provides ongoing monitoring of patient involvement, participation and compliance with treatment plan and goals as needed
- The Social Worker will perform in accordance with the State of New Jersey's regulations for Social Work
