About This Position

SOCIAL WORKER/CARE COORDINATOR - GERI PSYCH

  • POSITION SUMMARY
    • The Social Worker is responsible for providing direct high quality psychotherapy services to patients in assessments, diagnoses, treatment planning, group treatment; individual treatment, family treatment, and discharge planning.
  • POSITION QUALIFICATIONS
    • Minimum Education
      • A Masters Degree or above from an accredited college with a major in Social Work (MSW)

    • Minimum Experience
      • N/A

    • Required Registration/License/Certification
      • Must have a valid state license in Social Work as required by the state in which the Social Worker will be providing services (LMSW, LICSW or state equivalent). Must be obtained within 12 months of hire/transfer date.
      • BLS within 90 days of DOH/Transfer
      • CPI must be obtained within 90 days of hire date.

    • Preferred Education
      • N/A

    • Preferred Experience
      • Minimum three years' of experience in a similar position and/or industry

    • Preferred Registration/License/Certification
      • N/A

    • Other Requirements
      • N/A
