We are actively seeking a Director of Rehabilitation (DOR) to lead and grow our therapy program in a fast-paced skilled nursing setting. This is a hands-on leadership role for someone ready to take ownership, build a strong team, and drive both clinical outcomes and program growth.

What You’ll Do
- Lead and manage all therapy services (PT, PTA, OT, SLP)
- Hire, train, and supervise therapy staff
- Drive caseload growth and program development
- Ensure compliance with Medicare, Medicaid, and regulatory standards
- Oversee scheduling and ensure proper coverage
- Partner with nursing and IDT to coordinate care plans
- Participate in care planning, discharge planning, and triple check
- Monitor outcomes and implement improvements quickly
- Build relationships with hospitals and referral sources
What You Need
- Licensed Physical Therapist (PT) or PTA in Oregon
- Minimum 3 years of rehab/longterm care experience
- Leadership experience (DOR, ADOR, or Lead Therapist)
- Knowledge of PPS, PDPM, and skilled nursing operations
- Strong leadership, communication, and problem-solving skills
What Makes You Stand Out

- Experience with MDS, care planning, and reimbursement processes
Requirements:
- Current CPR Certification
- Valid driver’s license
- Must pass background check
Pay: $105,000 - $130,000 per year
