LPN / Wellness Director at Behavioral Health Assisted Living at Magnolia Operations LLC – Spokane, Washington

We are seeking a dedicated LPN to join our healthcare team. The ideal candidate will be a leader in the building, managing the nursing department, and provide specialized care for patients with mental health disorders in the assisted living setting.

Duties

• Monitor residents’ physical, mental, and behavioral status
• Oversee daily health services within the facility to ensure resident needs are met
• Coordinate medication management, refills, pharmacy communication, and MAR accuracy
• Provide direct nursing care including vitals, wound care (per scope), and health monitoring
• Support nurse delegation processes by coordinating with the RN
• Maintain accurate and timely documentation of resident health information
• Communicate effectively with families, physicians, pharmacies, and care teams
• Participate in resident care planning and follow established care plans
• Provide oversight and guidance to caregivers regarding resident health needs
• Assist with new resident admissions, reviewing medical histories and service needs
• Support behavioral plans created by Behavior Support staff and report any behavioral changes
• Assist in training caregivers on basic health, medication, and safety procedures
• Promote a safe, clean, and supportive environment for residents

Qualifications
- Valid LPN license in WA
- Previous behavioral health experience preferred
- Knowledge of psychiatric medications and treatment modalities is a plus
- Strong communication and interpersonal skills
- Ability to manage multiple priorities effectively
- Familiarity with electronic health record systems is a plus
