Behavioral Health Clinic Nurse (LPN) at Konza Prairie Community Health Center, Inc – Junction City, Kansas

Make a real impact in behavioral health, primary care, and substance use (MAT) at Konza Prairie Community Health Center. We’re hiring a compassionate LPN to support providers, coordinate care, and improve patient outcomes in a fast-paced, team-based clinic.

Key Responsibilities

• Patient intake, rooming, vital signs, and assisting with exams/treatments
• Nursing triage (phone calls, symptom assessment, urgent vs. non-urgent)
• Administer medications, injections, and support treatment plans
• Coordinate referrals, prior authorizations, diagnostic testing, and follow-ups
• Support behavioral health and substance use disorder (MAT) patients
• Monitor medication adherence, compliance, and relapse prevention needs
• Manage prescriptions and refills with pharmacies
• Document accurately in EHR/EMR (eClinicalWorks)
• Provide patient education on medications and care plans

• Active, unencumbered Kansas LPN license
• BLS (Basic Life Support) certification
• Vaccinations per ACIP healthcare guidelines

Preferred Qualifications

• Experience in behavioral health, mental health, substance use disorder, MAT, or primary care
• Experience with triage, care coordination, or case management
• Bilingual (English/Spanish)
