The Health Services Navigator provides support, education, and advocacy to people living with HIV (PLWH), helping empower them to develop effective strategies for living healthy lives. Through one-on-one interactions, the Navigator supports patients/clients in navigating medical care and addressing social drivers of health.
Duties & Responsibilities1. Advocacy and Support- Assists Case Managers and Ryan White team members during interventions and educational sessions.
- Through one-on-one interactions, promotes engagement with health care and provides opportunities for education, skill-building, and emotional support in a respectful and inclusive setting.
- Provides education and information to support patients/clients in accessing health information.
- Provides ongoing support and advocacy to help enhance coping skills, self-determination, and self‑advocacy.
- Coaches patients/clients in self-management and self-advocacy.
- Collaborates with supervisors, Case Managers, and interdisciplinary staff to address patient/client needs.
- Schedules appointments on behalf of patients/clients and assists with appointment adherence.
- Builds relationships with patients/clients and interdisciplinary team members to support continuity of care.
- Assists patients/clients in connecting to needed resources and maintaining services.
- Supports adherence to treatment recommendations.
- Documents each patient/client interaction comprehensively and concisely.
- Adheres to all government and funder regulations.
- Participates in case conferences, program meetings, and staff meetings as applicable.
- Promotes ethical practice, boundaries, and confidentiality in all peer support activities.
- Conducts outreach and engagement efforts to support engagement and retention in care.
- Links lost‑to‑care patients/clients to services through targeted outreach.
- Coordinates transportation, attends appointments when appropriate, and conducts home visits as needed.
- Promotes patient/client independence by teaching transportation systems and scheduling medical transportation.
- Collaborates with community providers on patient retention.
- Participates in “Patient Retention” meetings, provides updates, and reviews outreach assignments.
- Demonstrates knowledge of community resources.
- Demonstrates the ability and commitment to respect and support patients, employees, and visitors with diverse lived experiences.
- Understands, documents, and works across grant requirements; provides coverage for Health Navigation Services as needed (RWBSS, RWC, ARCH/PAWH).
- Demonstrates commitment to the mission, vision, and One Trillium values and models them in the organization.
- Maintains confidentiality of all protected patient, client, employee, and proprietary information.
- Meets department performance standards and participates in compliance audits, process improvement, and quality improvement plans.
- Participates in ongoing improvement of job responsibilities and organizational processes.
- Other specific duties as assigned.
- High School Diploma or equivalent preferred.
- Ability to read, speak, and understand English fluently.
- Experience in peer support or related roles.
- Experience facilitating support groups.
- Familiarity with community resources and social service systems.
- Strong interpersonal and communication skills.
- Ability to build rapport with diverse populations.
- Empathy and understanding of challenges faced by the client population.
Physical Requirements:
While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
