OVERVIEW:
Under the supervision of the Housing & Community Service Director. This position will
require helping the community with permanent housing. This is a regular full-time, non-exempt position.
ESSENTIAL FUNCTIONS:
- Identify and help secure appropriate rental housing in the community for rapid rehousing clients
- Provide housed clients with support to sustain housing long-term
- Monthly case evaluations to determine ongoing need for assistance
- Develop, establish, and maintain professional working relationships with landlords and property managers
- Guide and educate clients on how to search for, secure, and maintain permanent affordable housing, including tenant/landlord rights and responsibilities
- Assist clients with the completion of all pertinent documents to address their housing needs
- Assist clients in applying for mainstream public benefits and connecting them to resources
- Maintain thorough and accurate progress notes, files, and correspondence in HMIS
- Coordinate, participate, and assist with outreach events
- Conduct client intakes and ensure all supporting documents are verified and on file.
- Maintain timely case management files for all clients.
- Monitor and document the progress of clients.
- Develop, perform, and assist with workshops about clients’ needs.
- Attend monthly meetings and workshops about homeless services
- Develop and maintain contacts/relationships with community resources and partners
EXPERIENCE AND KNOWLEDGE:
- Minimum of three years working for a public agency with demonstrated ability in performing required job functions.
EDUCATION/CERTIFICATIONS:
- Minimum AA degree
LICENSES AND OTHER REQUIREMENTS:
- Ability to communicate, read, write, and follow oral and written instructions in Spanish/English, and able to communicate
- effectively and professionally with farm workers and their families.
- Valid California driver’s license, reliable transportation, good driving record, and proof of minimum California vehicle insurance
- Ability to walk, stoop, and bend as needed; ability to lift and carry up to 25 pounds and sit for long periods
