Our Case Managers supports the mission of the Community Action Agency by providing family-centered, results-oriented services designed to reduce poverty and promote self-sufficiency. Case Managers serves as a primary access point for individuals and families seeking services, assesses client needs and barriers, coordinates internal and external resources, and advocates for clients to achieve economic stability and improved quality of life.
Case Managers will:
* Conduct Client Intakes (virtually or in-person)
* Evaluate the individual's or family's situation to find the best pathway forward in their journey to self-sufficiency
* Regularly meet (virtually or in-person) with your Case Management Clients to assess where they are in the journey
and find or provide any additional resources they need for that journey
Each Case Manager covers three counties and spends 75% of their time working with clients within their portfolio of clients, either virtually such as on Zoom calls, or in-person, as needed. The rest of their time is spent working with community leaders and partners to determine the needs in those counties and how we can be a partner in meeting the needs of that community.
The successful candidate for this position will have the ability to work from home and/or work from an office space, as needed to service the needs of the clients we serve.
This position will be full time, non-exempt, and will have a company vehicle for when they do need to travel around counties. This position will cover Doddridge, Wetzel and Tyler Counties in West Virginia.

Requirements:High School Diploma or GED. Associate’s Degree in social services, or related field, required. Minimum of five years’ experience in non-profit setting and in assisting clients in crisis. Must have a good working knowledge of Microsoft Word and Excel and basic understanding of database programs. Proven experience in managing own workflow and coordinating processes.
GENERAL REQUIREMENTS:
Good oral and written communication skills, ability to work well under pressure, self-starting initiative, ability to handle multiple tasks simultaneously, ability to work one-on-one with individuals and in a group setting . Must be able to work with minimal supervision. Must be able to understand and implement written work plans. Must be able to complete and submit reports in a timely and accurate manner.
