Position Overview

The Case Manager is a fast-paced, hands-on position requiring strong interpersonal skills, creativity in problem-solving, flexibility, and excellent self-management. This role works extensively in the field throughout Riverside County, supporting youth and young adults (ages 18–24) experiencing homelessness, including former foster youth and other highly vulnerable populations. Services are provided in scattered-site housing, community settings, and non-traditional environments, often requiring adaptability and comfort working in unconventional situations.

Under the direction of the Transitional Housing Manager, the Case Manager provides comprehensive client advocacy, case management, housing navigation, and workforce development services. The position integrates housing stabilization with employment support to help participants secure safe housing, increase income, overcome barriers, and achieve long-term self-sufficiency.

The Case Manager administers rental assistance based on individual need and supports participants facing unique challenges. Through individualized service planning, the Case Manager helps each participant develop clear housing and employment goals that address barriers and promote stability.

Employment navigation is a key component of this role and requires a high level of detailed documentation, file management, and follow-through. The Case Manager works collaboratively with participants to develop individualized employment plans focused on increasing income and sustaining competitive employment. This includes identifying barriers, coordinating job leads, assisting with training and education enrollment, supporting interview preparation, addressing background challenges, and providing ongoing encouragement and accountability.

Collaboration with employers, community agencies, and internal team members is essential to creating a coordinated support network that promotes personal growth, independence, and successful long-term outcomes. The Case Manager must demonstrate a strong commitment to Housing First principles, trauma-informed care, advocacy, and a nurturing, strength-based approach that fosters resilience and empowerment among the youth served.

Essential Functions

  • Conduct intakes, interviews, screenings, and comprehensive assessments.
  • Develop individualized case plans that incorporate housing, employment, education, and personal development goals.
  • Provide ongoing case management including crisis intervention, budgeting assistance, referrals, housing placement support, and stabilization services.
  • Utilize Housing First principles to assist youth in obtaining, moving into, and maintaining permanent housing.
  • Provide early intervention when housing issues arise and support participants regarding tenant rights and housing responsibilities.
  • Collaborate with Housing Navigators and community partners to facilitate housing placements.
  • Administer rental assistance in accordance with program guidelines and participant needs.
  • Provide referrals and connections to mental health services, healthcare, substance use treatment, childcare, and other supportive services.
  • Assist participants in accessing mainstream benefits (e.g., SNAP, DPSS, Medi-Cal, Social Security).
  • Maintain accurate and timely documentation including case notes, service transactions, assessments, and HMIS data entry.
  • Prepare reports and maintain compliance with grant and administrative requirements.
  • Facilitate weekly workshops and life-skills groups.
  • Provide outreach and engagement to newly referred or system-matched youth.

Requirements:

Education Qualifications and Requirements Include

  • Associate or bachelor’s degree in social work, Social Sciences, Psychology, Human Services, Workforce Development, or related field required (or equivalent experience).
  • Minimum of two (2) years of experience in case management, workforce development, or related human services field preferred.
  • Experience working with youth experiencing homelessness, former foster youth, or chronically unemployed individuals preferred.
  • Knowledge of Housing First, Trauma-Informed Care, Harm Reduction, Motivational Interviewing, and Positive Youth Development.
  • Understanding of adolescent development and crisis intervention.
  • Knowledge of public benefits systems and housing subsidies (e.g., Section 8, VASH, Shelter Plus Care) preferred.
  • Strong understanding of employment readiness, job placement strategies, and workforce systems.
  • Experience building relationships with diverse communities, employers, and service providers.
  • Strong written and verbal communication skills.
  • Proficiency in Microsoft Office (Word, Excel) and database systems; HMIS experience preferred.
  • Ability to manage confidential information responsibly.
  • Bilingual Spanish-speaking ability is a plus.
  • Demonstrated ability to work independently and collaboratively.
  • Strong problem-solving, conflict resolution, and organizational skills.
  • Highly motivated self-starter with the ability to manage multiple tasks in a high-pressure environment.
  • Working knowledge of generational poverty dynamics and social class barriers to employment and housing stability.

Requirements Include

  • Reliable personal (not public) transportation available for occasional errands. Mileage will be reimbursed.
  • Must possess a valid California Driver's License, have a minimum of three (3) years of licensed driving experience, and maintain an acceptable driving record as verified by the DMV.
  • Proof of Automobile insurance.
  • Employment eligibility verification.
  • Successful completion of background screening.

Work Schedule Requirements

  • Work schedules are subject to change or start and end times may vary based on business needs and operational requirements.
  • Flexibility and adaptability are essential to meet the demands of the role and the organization.

Physical & Mental Demands

Case Managers are often required to visit participants in non-conventional locations, sometimes while carrying supplies and needed equipment. This may include:

  • Walking extensive distances
  • Walking on unlevelled terrain
  • Carrying up to 30 lbs for extensive distances
  • Ability to multi-task and stay focused with many interruptions
  • Ability to stay composed in difficult situations
  • Regular travel within Riverside County required
  • Case Managers are also required to work for many hours out of their vehicles making many stops throughout the day.
