The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Case Management Social Worker will actively participate in initiatives that improve key performance indicators, such as length of stay, throughput, that align with RWJBH System Goals and ensure continuity of care. The Social Worker will provide clinical social work interventions, including supportive counseling, crisis intervention, and psychotherapeutic approaches that address psychosocial distress arising from illness, grief and loss, trauma, or other major life events.

Qualifications:

Required:

  • Graduation from an accredited graduate school of Social Work with an MSW (Master’s in Social Work) degree

Preferred:

  • Case Management certification
  • One or more years of experience in an acute hospital setting

Certifications and Licenses Required:

  • State licensure required by the NJ State board of social work examiners
  • Licensed social worker (LSW) or licensed clinical social worker (LCSW) depending upon years of social work experience and clinical course work
  • LSW may render clinical social work services under the direct supervision of an LCSW

Scheduling Requirements:

  • Day Shift

​​​​​​​Essential Functions:

  • Conduct comprehensive assessments in the electronic health record
  • Complete timely referrals for post-acute care services using electronic referral systems
  • Ensure compliance with CMS Conditions of Participation, HIPAA, and organizational policies
  • Participate in interdisciplinary rounds, presenting cases and communicating discharge planning updates
  • Develop and implement individualized clinical interventions to support safe discharge, reduce readmissions, and coordinate care
  • Utilize health risk assessments and Social Determinants of Health screenings to identify at-risk patients and coordinate appropriate referrals
  • Demonstrate working knowledge of utilization management, level of care criteria, and payment models
  • Collaborate with the Utilization Review team to support insurance authorization and clinical documentation needs
  • Proactively manage patient flow by identifying discharge barriers and escalating complex issues to appropriate leaders
  • Address healthcare-related social needs by assessing and coordinating appropriate SDOH services to support safe discharges
  • Participate in ongoing professional development, quality improvement, and department-wide initiatives focused on LOS reduction, performance, and engagement
  • Conduct specialized psychosocial assessments, when indicated, including hemodialysis, transplant, perinatal, and oncologic
  • Provide evidence-based clinical social work interventions:
    • Clinical social work practice in the acute care setting addresses the intra- and interpersonal dynamics of illness, survivorship, life transitions, and end-of-life, with attention to how identity, roles, and systems are reshaped by serious illness
