SOCIAL SERVICES DIRECTOR at Auburn Village LLC – Auburn, Indiana

Auburn Village in Auburn, IN Seeks a Dynamic Social Services Director! This is a salaried position offering between $55k-$62k, negotiable based on level of experience in skilled nursing.

The Social Services Director assists with the planning, developing and directing our facility’s social service programs in accordance with current federal, state and local standards, guidelines and regulations. Most important is the ensuring that we are meeting not only the medical needs of each of our residents but that each resident’s emotional and social needs are met.

Hours are 8:00a - 4:30p (flexible).

Responsibilities of the Social Services Director:

• Greet and orient new residents
• Complete relevant Social Services Assessments
• Manage Care Plans
• Facilitate regular care plan meetings with residents and their families
• Provide information regarding community resources to residents' families
• Coordinate hospice and palliative care
• Coordinate discharge planning
• Complete the MDS (Minimum Data Set) within the Assessment Reference date
• Address residents' grievances and assist nursing or other pertinent departments in resolving issues
• Attend/facilitate intervention, rehabilitation, marketing, performance improvement and Medicare meetings as appropriate
• Ensure that all Federal, State and Local standards are maintained

· Must be have a Bachelor or Master's Degree in Social Services

· A member of the National Association of Social Workers (NASW) and Academy of Certified Social Workers (ACSW) - Preferred

· Minimum two years' experience as a social worker in Long Term Care (LTC)

· Demonstrated knowledge of the social dynamic of the geriatric population

· Knowledge of community resources

· Strong leadership and relationship building skills

· High level of organizational professionalism

· Working knowledge of government regulations as relevant to social services

· Initiative to seek new methods and services for residents
