Job Description
The Food Access Coordinator, under the Director of Nutrition and Lifestyle Programs, is responsible for developing, implementing, and delivering nutrition education and policy, systems and environmental change interventions that increase access to fresh food, with a focus on supporting limited-income community members throughout the Zufall Health service area. The Coordinator will develop and manage multiple food security initiatives focused on building partnerships that increase food accessibility. Most importantly, the FAC supports the mission and vision of the Zufall Health.
Essential Functions, Duties and Responsibilities

- Keeps supervisor always informed of work-related issues.
- Performs additional duties as determined by supervisor.
Specific Duties:
Promotion:
- Participate in organizing special events that seek to promote Zufall Eats and Zufall Health programs.
- Develop relationships with community leaders, public health staff, hospital contacts, and others who will refer participants or provide resources to participants.
- Represent Zufall Health at meetings with various organizations outside of Zufall Health.
Day to Day Operations:
- Develop and implement food access education, policy, and environmental change interventions that increase access to fresh food in targeted populations. Such programs may include, but are not limited to:
- Food is Medicine Initiatives
- Fruit and Vegetable Prescription Program (FVRx)
- Summer Meal Service Program (SFSP)
- Engage with program target populations via surveys, focus groups, and conversations to support program participation, evaluate programs and ensure they support the community needs.
- Collaborate with community partners, such as food pantries and food access organizations to build and maintain partnerships to support Zufall Health patients and the community.
- Analyze data collected for the food security program generate reporting of program metrics to key stakeholders.
- Maintain and update project plan in support of project operations.
- Prepare reports and documents as needed or requested.
- Responsible for driving the Zufall Eats van
- Creates the schedule of the van visits and events by working in conjunction with agency representatives, outreach staff and nutrition team.
- Oversee the mobile van regarding maintaining cleanliness and organization on the van at all times. Coordinate cleaning schedule with cleaning company.
- Ensure that the needs of the van are attended to in regard to daily upkeep and regular maintenance by developing and maintaining a maintenance record.
- Communicate in advance with any staff or team member that will be assigned on the van regarding any events or cancellation of events.
- Ensure in advance that the address for the event is clear, and the route to the event is accessible for the van in regards to bridge clearance. Coordinate with the site contact that there is sufficient parking space and access for the van.
Facilities Management
- Maintain vendor contact lists for van for emergency circumstances and coordinate service as needed with supervisor.
- Monitor mobile sites' appearance and maintenance and submit request to supervisor for regular maintenance issues and repair needs.
Other:
- Participate in regular team meetings and keep Zufall Eats calendar updated on One Drive.
- Attend off site meetings or conferences which may be necessary for training or that may enhance job performance.
- Other tasks, duties, or responsibilities as needed and requested.
Communication
- Maintains clients' confidentiality in compliance with HIPAA and other federal, state and local regulations as stated in the Zufall Health policies and procedures manual.
- Responds to all client issues and complaints, in person or via telephone regarding regulations and services; when necessary, refers inquiries to appropriate person or department.
- Reads, writes, speaks, understands, and communicates in English sufficiently to perform the duties of this position.
- Have strong knowledge of computer software programs such as Microsoft Word and Excel
- Have excellent customer service skills.
- Be organized and an independent worker.
- Be able to adapt quickly to unanticipated changes in workflow or work process.
- Have excellent verbal and written skills.
- Knowledge of HIPAA guidelines.
- Bilingual English and Spanish.
- Clean driving record.
- Flexible schedule.
