Job Responsibilities:

• Assist the men with scheduling and coordination of health care specialists
• Accompany Jesuits to physician or specialist appointments as needed
• Report changes in condition and relevant health updates to the nurse on duty, Care Coordinator Nurse, and Jesuit leadership
• Manage admissions and readmission, including communication of condition, treatments, and medications
• Ensure care plans are accurately and promptly communicated to the Care Coordinator Nurse and/or floor nurse on duty
• Assume full responsibility for assigned cases
• Complete documentation, charting, and planning in accordance with prescribed procedures

• Current Registered Nurse (RN) license in the State of Missouri
• CPR certification required
• Minimum of one (1) year experience in an adult medical/surgical unit
• Six (6) months of home care experience preferred
