Manage acute and chronic medical conditions in older adults with an emphasis on appropriate workup and diagnosis aligned with patient goals and values.
Elicit and document goals of care, advance care planning, and shared decision-making conversations with patients and caregivers.
Perform consultative evaluations in the hospital, outpatient clinic, assisted living facilities, and patient homes as needed.

Ensure documentation and care planning are consistent with regulatory standards and value-based care principles.
Collaborate with supervising geriatrician for case discussions, clinical oversight, and professional growth.

Qualifications:

Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) with prescriptive authority. Licensure in New York State prior to commencement of credentialing.
Certification and clinical experience in geriatrics, internal medicine, family practice, or related specialty strongly preferred.
Strong interpersonal and communication skills with patients, families, and interdisciplinary teams.
Demonstrated expertise in geriatric syndromes, polypharmacy, functional assessment, and cognitive evaluation.
Comfort with home visits, assisted living consultations, and transitions of care.
Commitment to patient-centered and value-based care models.

Strengths of the Role:

Longer, unhurried visits that allow for meaningful relationship-building and comprehensive care.
Opportunity to practice across multiple care settings with flexibility and variety.
