Responsibilities
- Work under the direct supervision of a pharmacist to support patients throughout various phases of care.Collect the best possible medication history to support accurate and timely medication reconciliation by the care team.Facilitate the provision of discharge medications, helping ensure patients have access to necessary medications upon leaving the hospital.Collaborate closely with pharmacists, nurses, physicians, and other care team members to ensure safe and effective medication use during transitions of care.Support efforts to reduce medication-related readmissions and improve continuity of care.
Qualifications
Education:
- High school diploma or equivalent required.
- Graduate of a pharmacy technician program preferred.
- Minimum 2 years of experience as a pharmacy technician required; retail and/or inpatient hospital experience preferred.
- Experience applying customer service behaviors and communication skills required.
- Basic familiarity with computers and keyboard skills and ability to navigate electronic systems applicable to job functions.
