About This Position
Job description
Infinity Care Services, Inc. in Stockton, CA is seeking Speech Therapists (ST)/Speech Language Pathologist (SLP) to work with our clients, clients families and health care providers to provide hands-on care and participates in the development and periodic review of the Plan of Treatment. We are looking for Therapists in San Joaquin and Stanislaus Counties. Together with our clients, clients families and health care providers, our Speech Therapists will:
Perform speech therapy assessments, diagnostic tests, skilled treatments and ongoing evaluation for clients who are receiving services under a medically approved Plan of Care. Implement teaching appropriate to the clients needs and abilities, and evaluate the effectiveness of the plan. Develop care plan guidelines for other members of the team to support the client goals. Refer to other disciplines as indicated and document accordingly. Supervise home health aides when providing services related to the speech language therapy care plan. participate in interdisciplinary activities per Agency guidelines. Perform job in compliance with Agency policies and procedures and professional and community standards. Promote personal safety and a safe environment for clients and coworkers. Maintain confidentiality in all aspects of the job. Perform other related duties and responsibilities as deemed necessary. Qualifications:
Meets the education and experience requirements for Certificate of Clinical Competence (CCC) in speech pathology or audiology granted by the American Speech-Language-Hearing Association (ASLHA), or has completed the educational requirements for certification and is in the process of accumulating the supervised experience required for certification. Is certified to practice Speech-Language Pathology. Has a minimum of one (1) years; experience in a health care facility or equivalent experience. Home Health experience a plus Current CPR certification Demonstrates strong verbal and written communication skills with good interpersonal skills. Licensed driver with automobile insured in accordance with state Job Types: Part-time, PRN, Per diem
