Services combine health monitoring, social engagement, and daily support in a structured setting. Position Summary The Activities & Nutrition Lead is a dual-role position responsible for planning, coordinating, and overseeing both life-enrichment programming and nutrition services within the Golden Years ADHC center in alignment with Kentucky adult day health regulations and Medicaid/HCB Waiver standards. This position serves as the designated staff member primarily responsible for dietary and activity services, as contemplated in Kentuckys adult day health regulations. The Activities & Nutrition Lead ensures participants receive safe, meaningful, person-centered activities and meals that support their health, dignity, and ability to remain in the community. About This Specific Position This is a hands-on, relationship-centered role. You will spend your day leading and coordinating group and individual activities while also making sure the daily menu, meal service, and nutrition practices meet regulatory expectations and participant needs. You will work closely with nursing, case management, kitchen staff, and direct care staff so that meals are on time, activities are meaningful, and documentation is compliance ready. Key Responsibilities Activities / Life Enrichment Plan, implement, and evaluate a structured calendar of daily activities (social, recreational, cognitive, physical, and creative) that support participant abilities, preferences, and care goals. Adapt activities for participants with varying cognitive and physical abilities, ensuring safe and meaningful engagement. Lead group sessions and provide 1:1 engagement for participants who benefit from more individualized support, focusing on reducing isolation and promoting emotional well-being. Coordinate special events, seasonal programs, and community engagement opportunities that support HCB Waiver goals of community-based services and inclusion. Collaborate with case managers and nursing to align activities with person-centered service plans and any therapy/behavioral recommendations. Guide and support direct care staff and volunteers in activity delivery, participant engagement, and safety practices. Key Responsibilities Nutrition / Dietary Services Coordinate daily meal and snack services in accordance with Kentucky adult day health regulations and applicable nutrition standards for congregate settings. Partner with contracted or consulting nutrition professionals (as applicable) in menu planning that incorporates participants medical, cultural, and personal dietary needs. Ensure menus and acceptable substitutions are followed as planned, with attention to portioning, texture modifications, and special diets as ordered by nursing or a nutrition professional. Oversee safe food handling and service practices, reinforcing procedures consistent with recognized food safety standards such as those reflected in nutrition program guidance. Communicate clearly with kitchen/food service staff on timing, presentation, and participant-specific needs (allergies, swallowing risks, diabetic needs, etc.). Maintain required records related to menus and food service as directed by internal policy and regulation. Key Responsibilities Compliance, Documentation & Safety Operate activity and nutrition services in compliance with Kentucky adult day health licensure regulations and HCB Waiver expectations for adult day health services. Maintain documentation for activities (calendars, attendance, adaptations) and nutrition (menus as served, special diet instructions) in line with policy and survey readiness. Participate in internal quality improvement, including preparation for surveys, audits, and waiver reviews that focus on programming and nutrition. Identify and report incidents, adverse events, or safety concerns quickly, assisting with follow-up and preventive actions as needed. Coordinate with nursing and case management when participant health or behavior changes indicate the need to adapt activity or dietary approaches. Schedule & Work Environment Full-time, center-based position with a consistent schedule: Monday through Friday, 6:00 AM 2:00 PM. Work occurs primarily within the adult day health center, with regular interaction with participants, staff, and occasionally families or community partners. The role includes standing, facilitating groups, light lifting/room setup, and coordination with kitchen/food service areas.

Compensation, Training & Performance Structure Pay band: $18.00 $22.00 per hour, based on experience, certifications, and leadership skills. Training: Structured hip-to-hip onboarding with experienced team members in both activities and nutrition, including weekly feedback during ramp-up to support knowledge retention, skill application, and confidence. Performance reviews: Formal 6month performance review followed by an annual review on your anniversary date. Reviews are based on a set of clear KPIs, discussed openly during orientation and reinforced during handoff so expectations are transparent and fair. Probationary period: 90 days to confirm reliability, cultural fit, quality of programming, and adherence to Medicaid/HCBW and licensure requirements. Ongoing development: Monthly 1:1 check-ins with leadership, targeted coaching, and ongoing education on person-centered practices, adult day health and HCB Waiver standards, safety, and best practices in activities and nutrition. Qualifications & Skills Experience in activities coordination, recreation therapy, social services, nutrition support, or related roles serving older adults or adults with disabilities is preferred. Familiarity with, or strong interest in learning, Kentucky adult day health regulations and HCB Waiver expectations for adult day health services. Demonstrated ability to plan and lead engaging activities for individuals with varying functional and cognitive levels. Basic understanding of nutrition and safe food service in a congregate setting; formal training or credentials in nutrition or food safety are a plus. Strong collaboration skills with interdisciplinary team members (nursing, case management, attendants, kitchen staff, and leadership). Organized and dependable, able to manage calendars, menus, documentation, and day-to-day logistics. CPR certification (or willingness to obtain) is consistent with expectations for staff involved in direct services in adult day health settings. Disqualifiers & PreEmployment Requirements Must successfully complete all required pre-employment screening, which may include: Criminal background check Drug screen TB test Must hold a valid drivers license and maintain current proof of auto insurance if driving is required for program responsibilities. Candidates unable to meet these requirements are not eligible for hire. Once a formal offer is extended, candidates will have 48 hours to accept the offer and complete required onboarding tasks; failure to do so may result in withdrawal of the offer.
