About This Position

Care Coordinator (Neural Care Navigator)

1. Onboarding & Relationship Management (All resumes to be in English and PDF Format)
Onboard dementia patients and caregivers to the NeuralCare platform and maintain up-to-date profiles. Establish a regular check-in cadence via in-app chat, phone/recorded calls, or in-person meetings when needed. Build and maintain trusting, longitudinal relationships with patients and caregivers. 2. Care Navigation & Coordination
Assess needs and concerns with empathy, patience, and a caregiver-first mindset. Act as the single point of contact for care needs; ensure timely follow-up and resolution. Connect patients to appropriate community resources and coordinate clinical referrals. Conduct proactive outreach to high-risk dementia patients/caregivers to prevent care gaps. 3. Platform & Data Utilization
Use NeuralCares AI-powered tools for note-taking, triage, and resource research (e.g., ChatGPT, Gemini). Support in-home setup, device connectivity, and basic troubleshooting. Monitor digital inputs from the NC in-home platform and align findings with clinical response guidelines. 4. Documentation, Quality & Compliance
Document exclusively within the NC platform to maintain strict HIPAA compliance. Submit internal tickets and contribute feedback to improve workflows and platform features. Ensure timely, accurate, and encrypted documentation of all interactions.
Required Qualifications Education
Background in Nursing or Social Work preferred.
Experience
Caregiving experience required, preferably with dementia patients. Experience working with the U.S. healthcare system and insurance processes. Prior roles in care coordination, population health, or patient navigation strongly preferred. Minimum 3 years working with patients in a clinical or customer-support setting. Experience with geriatric or dementia populations is a strong advantage.
Language
English fluency required (U.S. market). Spanish proficiency a plus as the program expands.
Core Competencies
High empathy, compassion, and professionalism. Patience and comfort handling repeated questions and caregiver stress. Understanding of dementia staging, ADLs, and behavioral symptoms (preferred). Strong organization, prioritization, and time-management skills. Excellent verbal and written communication; conflict-resolution skills. Ability to stay calm and composed in stressful situations. Familiarity with medical terminology (preferred).
