Job Description

Description:

Tempe, AZ | Full-Time | $112K–$135K/yr DOE

Make a Real Difference — One Patient at a Time.

At Arizona Home Care, we believe patients deserve exceptional pharmaceutical care in the comfort of their own homes. As a locally owned, mission-driven home health and infusion pharmacy serving the Valley, we bring clinical excellence directly to the patients who need it most.

We're looking for a skilled, compassionate Home Infusion Pharmacist to join our clinical team and play a central role in improving patient outcomes across the Valley.

If you're a pharmacist who thrives in a collaborative, fast-paced clinical environment and wants to do more than fill prescriptions — this is your role. New to home infusion? No problem. We provide full training and mentorship to set you up for success.

Pay & Benefits:

  • Salary: $112K–$135K/yr DOE
  • Medical, dental, vision & life insurance
  • Pet insurance
  • 401(k)
  • Up to 120 hours of PTO + 6 paid holidays
  • Free CEUs to support your professional growth
  • A fully stocked kitchen with coffee, snacks, and more
  • Stable, supportive office environment
  • Locally owned, mission driven organization

Schedule:

  • Full-Time
  • Monday–Friday, rotating shifts (9:30am–6pm and 10:30am–7pm)
  • On-call rotation with a required response time of within 30 minutes
  • Holiday rotation as scheduled

Key Responsibilities:

  • Independently perform all pharmacy tasks including compounding, pharmaceutical calculations, data entry, and general workflow — with hands-on proficiency in every function overseen or verified
  • Perform sterile and non-sterile compounding independently in compliance with USP 797 and USP 800 standards
  • Review and verify medication orders for accuracy, drug interactions, contraindications, and dosing
  • Monitor patient drug therapy, interpret lab data, and adjust care plans to optimize outcomes
  • Collaborate with physicians, nurses, and interdisciplinary care teams to coordinate patient care
  • Educate patients and caregivers on medication use, safe administration, and what to expect
  • Supervise pharmacy technicians and compounding personnel
  • Use effective strategies to achieve daily goals, including prioritization, delegation, deferral, and reassignment of duties
  • Demonstrate proficiency with pharmacy software and data entry systems
  • Ensure full compliance with HIPAA, OSHA, state Board of Pharmacy, and accrediting body standards
  • Participate in QAPI initiatives and support a culture of continuous improvement
Requirements:

Required:

  • Doctor of Pharmacy (PharmD) or B.S. in Pharmacy
  • Active, unrestricted Arizona Pharmacist license
  • 1–2+ years of pharmacist experience
  • Ability to independently perform compounding, calculations, data entry, and pharmacy workflow tasks
  • Proficiency with pharmacy software and data entry systems
  • Strong organizational and time-management skills with the ability to prioritize effectively in a fast-paced environment
  • Strong communication and patient-facing skills
  • Ability to participate in on-call rotation with a 30-minute response time and holiday rotation as scheduled

Preferred:

  • Home infusion or sterile compounding experience
  • BCSCP certification
  • Familiarity with USP 797 and USP 800 standards
  • Experience in a collaborative, interdisciplinary or home health setting
