Essential Function |
|
Assist primary health care providers in recognizing and treating medically related behavioral health and psychosocial problems by performing on-demand consultations and evaluations during clinic hours. |
|
Play a primary role in the workflow development, implementation and evaluation of BHI efforts. |
|
Train and support clinic staff and providers with regards to the BHI practices such as using motivational interviewing to increase motivation to make changes other and evidence-based interventions for primary care settings. |
|
Assess the clinical status of patients referred by primary care providers through brief consultative, behavioral-focused contacts and make recommendations regarding treatment and need for community resources. |
|
Work with primary care provider to refer cases to community mental health specialists and community resources as appropriate. |
|
Assist in the detection of high-risk and organizational priority patients in developing plans to prevent further psychological or physical deterioration. |
|
Assist in preventing relapse or morbidity in conditions that tend to recur over time. |
|
Teach patients, families, and staff about behavioral healthcare, prevention, and treatment enhancement techniques through the use of classes, support groups and individual encounters. |
|
Attend and participate in BH team meetings, BHI grant-related meetings & training, provider meetings, billing considerations, committee meetings and planning activities as appropriate. |
|
Participate in an integrated care team for patients experiencing pain and other chronic conditions and assist clinics in developing care teams. |
|
Participate in patient charting in the electronic health record as well as the development of integrated care plans. |
|
Assist clinics throughout the system in adopting BHI-related workflows. |
|
Provide consultations focused on functional evaluations, and make short-term behaviorally-focused treatment recommendations that apply patient strengths and resources to identified problems. |
|
Ability and willingness to provide educational opportunities for behavioral health students and students from other health-related disciplines. |
|
Work directly with providers to align prescribing patterns with the CDC guidelines. |
|
Ability to make quick and accurate clinical assessments of mental and behavioral conditions and make short term behaviorally-focused treatment recommendations and provide feedback to referring providers on a same day basis. |
|
Participate in training, implementation, and practice of telemedicine-delivered behavioral health interventions. |
|
Engage in trainings related to prescribing, SBIRT, substance use, Motivational Interviewing, brief interventions, etc. |
|
Ability and willingness to communicate and evaluate health conditions such as chronic pain, diabetes, sleep disorders, etc. using standardized instruments and documentation thereof. |
|
Conduct brief interventions that are simple, racially and culturally sensitive and supportable by other primary care team members. |
|
Assist with preparation and participate in site visits as needed. |

This job primarily operates in a professional office environment and routinely requires the use of standard office equipment. This job is primarily indoor, with few working hazards, and some community or patient exposure. Prolonged periods of sitting at a desk and working on a computer. While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel. The employee is frequently required to sit for prolonged periods; look at a computer screen for prolonged periods; reach with hands and arms; and talk or hear. The employee is occasionally required to stand for prolonged periods; walk; climb or balance; stoop, kneel, or crouch. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. |
