| Job Summary | This position will work closely with both inpatient and outpatient providers of the Advanced Heart Failure service to improve care coordination for the heart failure population. |
| Skills / Knowledge / Abilities | The ideal candidate for this position will have extensive disease-specific knowledge; the ability to work independently and think critically; will be well-organized and detail-oriented; and will have excellent communication skills. |
