About This Position

Description:

JOB SUMMARY

Under the direction of the Disease Management (DM)/High Risk Case Manager and Ambulatory Case Management (ACM) leadership, the Disease Management High Risk Coordinator supports the identification, outreach, monitoring, and follow-up of Disease Management (DM) and High-Risk members enrolled in the ACM Program. The role facilitates care coordination activities, patient engagement, data tracking, and communication with providers and interdisciplinary teams in alignment with health plan and organizational guidelines.

Requirements:

MINIMUM & PREFERRED QUALIFICATIONS

Education/Training

Minimum: High school diploma or equivalent

Preferred: Associate’s degree

Experience

Minimum: 2+ years of care management or healthcare coordination experience. 1+ years of utilization review experience.

Preferred: Experience supporting disease management or high-risk populations.

Any combination of educational and work experience that would be equivalent to the stated minimum requirements would qualify for consideration of this position.

Skills, Knowledge & Abilities

· Proficiency in Microsoft Office applications (Word, Excel, Outlook)

· Strong verbal and written communication skills

· Ability to engage and communicate effectively with diverse patient populations

· Working knowledge of care coordination and utilization review processes

· Strong organizational skills and attention to detail

· Ability to prioritize tasks and manage multiple assignments

· Ability to work collaboratively within an interdisciplinary healthcare team

· Understanding of confidentiality and HIPAA requirements

This position requires the ability to perform primarily sedentary work with extended periods of sitting, intermittent standing and walking, and occasional light physical activity such as lifting up to 10 pounds, bending, reaching, and climbing stairs. The role requires frequent hand and finger dexterity for telephone use and documentation, as well as the ability to drive as needed for work-related activities.
