Job Description

Description:

Shift: Day

Job Summary: To provide services in accordance with the policies and procedures as of the Home Care Department, under the direction of a licensed Physician and/or a Registered Professional Nurse.

Demonstrates Competency in the Following Areas:

· Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical condition.

· Demonstrates ability to perform a head-to-toe assessment on all patients and reassessments, as per policy, including pediatric, geriatric and the general patient population.

· Performs patient care responsibilities considering needs specific to the standard of care for the patient’s age.

· Demonstrates competency in all skills required for the agency including but not limited to colostomy care, foley care, feeding tubes, FSBS’s, to venipunctures, infusion therapy, infusion therapy device care, would care, and aseptic technique.

· Evaluates and regularly re-evaluates the nursing needs of the patient; initiates, develops, implements and makes necessary revisions of the patient’s plan of care. Assesses patient’s continual care needs. Plan of care addresses all problems identified in the assessment or provides evidence of a rationale for not doing so.

· Observes signs and symptoms and reports to the physician and/or other appropriate health professionals as often as needed, changes in the patient’s condition.

· Teaches, supervises and counsels patient and family regarding home nursing procedures and other care needs as appropriate to the patient’s condition. Utilizes agency education material appropriate.

· Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation within 24 hours of visit.

· Regularly reads and appropriately applies to practice communication/memos.

· Initiates diagnostic, preventive and rehabilitative nursing procedures as appropriate to the patient’s care and safety. Makes referrals to other disciplines as indicated by the needs of the patient or documents rational for not doing so.

· Requires minimal supervision and is self-directed.

· Actively participates and is involved in JCMH Home Care’s quality assessment and performance improvement program. Can verbalize own responsibilities and activities.

· Responsible for following all policies and procedures of the agency regarding service delivery, documentation, and care coordination. During supervisory visits, exhibits full compliance and verbalizes knowledge of agency policies, including but not limited to patient education, infection control, management of hazardous wastes, and age-specific behaviors for patients.

· Demonstrates knowledge of policies and procedures applicable to own job position and supports necessary changes. Is not detrimental to the harmony of the department.

Demonstrates Competency in the Following Areas:

· Interacts professionally with patient and family and involves them in patient teaching and plan or care.

· Attends departmental meetings and monthly interdisciplinary case conference and participates in a positive manner

· Assists nurse manager in the development and implementation of Performance Improvement activities to improve patient outcome and work environment.

· Identifies and addresses psychosocial needs of patients and family.

· Demonstrates ability to be flexible and organized; demonstrates ability to function well under stressful situations.

· Maintains strict confidentiality of patient information within and without the department. Respects patients and co-workers rights to privacy.

Requirements:

Professional Requirements:

· Completes annual educational requirements. Completes in-services and staff meetings and returns in a timely fashion.

· Maintains regulatory requirements.

· Reports to work on time and as schedules; consistently completes assigned duties and avoids excessive and/or unauthorized overtime. Is available when on call according to policy.

· Communicates the mission, ethics and goals of the hospital as well as the mission and vision of Home Care.

· Adheres to dress code and is professional in appearance and conduct.

· Adheres to agency infection control and safety policies, including education, reporting, and practice implementation specific to job position.

· Develops work relationships that honor and respect others’ strengths and abilities.

· Must be able to demonstrate knowledge of the principles of growth and development over the life span and to identify each patient’s requirements relative to his/her age-specific needs.

Education/Regulatory Requirements & Experience:

· Current RN licensure in State of Oklahoma required.

· Minimum of 1-year experience as RN preferred.

· Previous work history that demonstrates steady attendance and punctuality is required

· Current BLS required.

· Maintains current automobile insurance, current drivers license, and maintains driving record of good standing.

· CPR (BLS) can be obtained during orientation.

Language Skills:

· Ability to read and communicate effectively in English.

· Additional languages preferred.

Skills:

· Basic computer knowledge
